NOTE: If you have a surname that was in more than one county, please submit that surname separately under each county (for example, if you have Smith in Johnson and Marion counties, you would submit Smith in Johnson County and then submit Smith in Marion County).

• Includes – will search for that string of characters. Searching for Berry would yield Berry, Berryman, Millberry, etc.
• Exact – will search for that exact text only. Searching for Berry would yield only Berry.
• Soundex – will search for Soundex equivalents. Searching for Berry would yield Berry, Beery, Biery, etc.
